\n\u2022 Any reports or letters from other medical providers
\n\u2022 Forms to be completed by medical provider

\n\u2022 Income information if uninsured

<\/div><\/div>[vc_toggle title=”How can I pay for my visit?” style=”rounded” css=”” icon=”icon-plus-sign”]We accept Medicaid, Medicare, CHIP, and private insurance. Uninsured or underinsured patients may be eligible for our sliding fee scale. Co-pays, deductibles, co-insurances and fees for non-covered services are expected to be paid at the time of your visit. We accept cash, personal checks, Visa, MasterCard, American Express and debit cards. All patients are welcome regardless of their ability to pay. An estimate of the cost for medical services and items for patients who are not insured or who are not using insurance will be made available prior to the patient’s visit.[\/vc_toggle]
<\/div><\/div>[vc_toggle title=”How do I refill my medications?” style=”rounded” css=”” icon=”icon-plus-sign”]Medication refills can be requested by calling the health center or through the online patient portal. If leaving a message, please include the name of the medication you want refilled, the dose, and the pharmacy you want the medication sent to. Please allow for up to two (2) business days to process medication refill requests.[\/vc_toggle]

Who can fill the document out?” style=”rounded” css=”” icon=”icon-plus-sign”]If you have medical forms that you need filled out, please bring them with you to your appointment. Any forms brought in outside of your appointment time will take up to five (5) business days to complete.[\/vc_toggle]
<\/div><\/div>[vc_toggle title=”I need a referral to a Provider outside of MHEDS. How does that work?” style=”rounded” css=”” icon=”icon-plus-sign”]Referrals to specialty providers can be made during a visit with your MHEDS Provider. Please allow up to five (5) business days for our Referrals Department to complete the referral process. If you have any questions, you may call the health center.[\/vc_toggle]
