MHEDS offers Medical Case Management services through Anna Tischenko. Anna is MHEDS Medical Case Manager. She works to help patients and families navigate the health care system. She advocates for patients and interacts with healthcare team members to help us provide effective, efficient, and high-quality care. She also coordinates care and communicates patient needs to help guide the delivery of care. This is especially important for patients who have more than one health condition or need extra help to coordinate their health care with their health insurance and other supports. She is fluent in English and Russian.
Anna received her Medical Case Management certificate from the University of Southern Illinois, College of Nursing and Health Professions. She has been a refugee caseworker, social service support provider, English as a Second Language (ESL) volunteer teacher, interpreter, and translator in previous employment.
In addition to Medical Case Management, Anna was appointed to the Erie County Department of Health’s Community Advisory Board. She is also certified as a “Pennie” Assister. Pennie is Pennsylvania’s official Health Insurance Marketplace. Open Enrollment is from November 1, 2021, to January 15, 2022.
